I am aware that the programs and activities offered by the Canadian Sports Business Academy inc. or Endeavour Tours International Inc. Involve many inherent risks, Dangers and hazards and the possibility of personal injury, Death and property damage or loss.

These inherent risks, Dangers and hazards include, but are not limited to:

A）Weather conditions.

B）The use or operation of any vehicle, Vessel or other means of transportation.

C）The use or operation of any equipment associated with the programs offered or activities arranged, Guided or supervised by the Canadian Sports Business Academy inc. or Endeavour Tours International Inc.

D）The condition or state of repair of any facility, Equipment, trail, Walkway, pathway, Vehicle, Vessel or other means of transportation used in the programs offered or activities arranged, Guided or supervised by the Canadian Sports Business Academy inc. or Endeavour Tours International Inc.

E）The actions of any other student or other person, Including Canadian Sports Business Academy inc.'s or Endeavour Tours International Inc.'s instructors, Employees and agents, Whether or not those actions are negligent.

In consideration of my acceptance as a student in the Canadian Sports Business Academy inc. or my participation in programs or activities offered by or through Endeavour Tours International Inc., I AGREE:

1）To assume and accept all risks, Dangers and hazards in connection with my participation in the programs offered or activities arranged by the Canadian Sports Business Academy inc. or Endeavour Tours International Inc. and my use of any facilities during my participation in any such programs or activities, Including travelling to or from such facilities.

2）To waive any and all claims I may have against the Canadian Sports Business Academy inc. or Endeavour Tours International Inc., or their directors, Officers, Employees, Agents and representatives.

3）To release the Canadian Sports Business Academy inc. and Endeavour Tours International Inc. from.

　　Including negligence on the part of the Canadian Sports Business Academy inc. or Endeavour Tours International Inc., Breach of contract, or the breach of any statutory or other duty of care, Including any duty of care owed under the occupiers liability act, r.s.b.c. 1996, c. 337.

4）To hold harmless and indemnify the Canadian Sports Business Academy inc. and Endeavour Tours International Inc. From any and all liability for personal injury or death suffered by me or a third party, or property damage as a result of:.

5）That this release and indemnity agreement is effective and binding against my heirs, Next of kin, Spouse, Executors, Administrators and assigns.

6）In the event of a conflict or inconsistency between this document and its japanese translation (attached as schedule "a" of this document), This english document shall prevail.

I am over 19 years of age. I have read and understand this agreement prior to signing it. I am aware that by signing it I am waiving certain legal rights and accepting certain liabilities which I or my heirs, Next of kin, Spouse, Executors, Administrators and assigns may have against the Canadian Sports Business Academy inc. or Endeavour Tours International Inc.
